How To Apply
The applicant has to apply in standard form along with two attested copies of Birth/Death certificate issued earlier by this office of the person whose birth has to be reconstructed along with the affidavit and other supporting documents.
Process
On verifying of documents submitted the same is verified and verification report is forwarded to the office of Directorate of Municipal Administration for approval. On receipt of Order from the Office of Directorate of Municipal Administration, the re-construction of birth certificate is done.
