Birth / Death Certificate

How To Apply

Applicant has to give details in respect of name, place of Birth or Death & date of Birth or Death of the person whose Birth or Death certificate is required by him orally or in written.

  1. Applicant has to pay for a search to be made for extract as follows: –
  2. Search a single entry in the first year for which the search is made…………Rs.20.00
  3. For every additional year for which the search is continued…………………..Rs.20.00
  4. For extract relating to each birth or death ……………………………………………Rs.50.00

The Birth or Death certificate is issued on the spot.

Registration of Birth or Death    

  1. Registration of Birth or Death: –

Every Birth and Death is required to be registered within 20 days of its occurrence along with the following documents: –

  1. Form No.1 in case of Birth.
  2. Form No.2 in case of Death along with Medical Certificate in Form No.4.

Form No.4A duly signed by Medical Practitioner/Medical Attendant –with Registration Number.

NOTE: –

  1. In case if Birth and Death is registered after 21 days but not later than 30 days of its occurrence late fees of Rs.20/- will be charged extra.
  2. In case if the Birth and Death is not registered within 1 month of its occurrence the following documents are required to be attached: –
  3. Birth/Death order from the concerned Registrar.
  4. Fees of Rs.20/- is payable as registration fees in such case.
  5. In case the Birth/Death is registered after 1 month but within 1 year the following documents are required to be attached: –
  6. Birth/Death order from the District Registrar.
  7. The applicant has to pay Rs.50/- as fees.
  8. The certificates will be issued within 4 days after the Registration.
  9. In case the Birth/Death is registered after 1 year the following documents are required to be attached.
  10. Birth/Death order from the District Magistrate or Sub Divisional Magistrate or by an Executive Magistrate authorized by the District Magistrate.
  11. The applicant has to pay Rs.100/- as fees.
  12. a) Naming of the child shall be by the parents or any other person authorized by the parents of the child.

Process

Case I: Where the Birth/Death takes place in the Hospital.

The legal information and statistical information in form1 and form2 for births and deaths respectively are received from the hospital duly filled.

The duly filled form is submitted in the Registration of Birth and Death section of the Margao Municipal Council.

Upon registration, the Birth or Death Certificate is issued under section 12 free of cost and additional copy of Birth or Death Certificate is issued on payment of fees of Rs.50/- per copy.

 Case II: Where the Birth/Death takes place at Residence.

The form is to be collected from the Registration of Birth and Death section of the Margao Municipal Council.

The duly filled form to be submitted in the Registration of Birth and Death section of the Margao Municipal Council.

 Upon registration, the Birth or Death Certificate is issued under section 12 free of cost and additional copy of Birth or Death Certificate is issued on payment of fees of Rs.50/- per copy.
